School and sports physicals
Illinois requires a school health exam before kindergarten, 6th grade and 9th grade. Sports teams, park district leagues and camps need their own form. We do all of them.
What the visit includes
- Full head-to-toe physical exam
- Height, weight, blood pressure and pulse
- Vision and hearing screening
- Heart, lung, joint and flexibility check for athletes
- Review of past injuries and concussions
- Immunization record reviewed and updated
- Your school, sports or camp forms filled out and signed
How to book
Appointments are made by phone. Call (630) 784-8600 and tell us what the visit is for, or send the request form and we will call you back. Walk-ins are welcome. Calling first lets us have a room ready and keeps your wait short.
Monday–Thursday 11 AM – 5 PM · Friday 11 AM – 4 PM · Saturday 10 AM – 1 PM · Sunday Closed
Insurance
We accept Medicaid, All Kids and most major insurance plans. No insurance? Uninsured and self-pay families are welcome. Call us before the visit and we will check your plan with you.
Good to know
- Bring the form. Every school and league uses a different one, and we cannot sign what we do not have.
- Book early. June through August fills up, and forms are due before the first day.
- If your child is already due for a well-child visit, we can often do both in one appointment.
- Dental and eye exam forms are separate. Those are done by a dentist and an eye doctor.
Common questions
When does Illinois require a school physical?
Illinois requires a health examination before entering kindergarten or first grade, sixth grade and ninth grade, and for any student entering an Illinois school for the first time.
How long does a sports physical take?
Plan for about 20 to 30 minutes. Bring the form filled out as far as the parent section goes, and we handle the rest.
Can you sign a form for a physical done somewhere else?
We need to examine your child ourselves before signing. Bring records from the other office and we will review them at the visit.
