Teen visits

Teenagers need a different kind of visit than a toddler does. Yearly check-ups continue through age 18, with time for your teen to talk to the doctor directly.

What the visit includes

  • Annual check-up, growth and blood pressure
  • Private time between your teen and Dr. Farhana
  • PHQ-9 depression screening
  • Sleep, mood, stress and school questions
  • Sports physicals and school forms
  • Adolescent vaccines
  • Skin, period and puberty concerns
  • Referrals to counseling or a specialist when they would help

How to book

Appointments are made by phone. Call (630) 784-8600 and tell us what the visit is for, or send the request form and we will call you back. Walk-ins are welcome. Calling first lets us have a room ready and keeps your wait short.

Monday–Thursday 11 AM – 5 PM · Friday 11 AM – 4 PM · Saturday 10 AM – 1 PM · Sunday Closed

Insurance

We accept Medicaid, All Kids and most major insurance plans. No insurance? Uninsured and self-pay families are welcome. Call us before the visit and we will check your plan with you.

Good to know

  • Some of the visit is one-on-one with your teen. That is normal practice and it is how teens tell a doctor what is actually going on.
  • Teens who have been coming here since they were babies often prefer to keep seeing the same doctor through high school.
  • If your teen is anxious about the visit, tell the front desk when you book and we will take it slower.
  • Bring sports or school forms with you and we will finish them at the visit.

Common questions

Why does my teenager see the doctor alone for part of the visit?

Teens are more honest about sleep, mood, stress and risk when a parent is not in the room. Dr. Farhana will tell you anything that affects your child’s safety.

What is the PHQ-9?

A short, standard set of questions used to screen for depression. Teens fill it out at the visit. A high score is not a diagnosis, it is a reason to talk further.

Until what age can my child be seen here?

Through age 18. Many families stay until their child leaves for college.
